We are gearing up for our 3rd Annual Physical and ECG event again this year!  We are excited to announce that Physicals are ONLY $10.00/student instead of $20.00; special thanks to the provider!  Please print all the forms, front and back. Please fill out the front medical history form and ECG form if you are interested in your student participating.  Physicals are a requirement for ALL students in grades 7th, 9th, 11th who are participating in UIL extracurricular activities.  These will be performed during the school day and students from BIMS will be transported by bus to the BHS fieldhouse.  It is recommended that students receive an ECG every year they are required to have a physical.  If you have any questions please contact your campus nurse, Coach Demond, or Whitney Lamance – Director of Health Services.
